The Organization The Group Health Centre is a progressive multi-specialty ambulatory care health care organization and is one of Canada’s first consumer-sponsored health care facilities. As Ontario’s largest and longest-established alternatively funded healthcare organization, Group Health Centre provides primary care to over 50,000 Sault Ste. Marie and Algoma District residents, as well as various clinical programs to our entire community. In addition, Group Health Centre’s recently established the Access Care Clinic, led by Nurse Practitioners. The clinic provides access to essential healthcare services to recently de-rostered patients in the community as a temporary solution to current Primary Care Physician recruitment challenges. Position Overview As an integral member of our innovative interdisciplinary Primary Care Support Team, the Senior Social Worker will act as a subject matter expert and support the Department Manager in providing over-all direction and on-going development of the program as well as maintaining work systems, procedures, and policies that enable and encourage the optimum performance of day-to-day clinical coordination of services. The Senior Social Worker will actively collaborate on healthcare initiatives, utilizing evidence-based practices to support patients and families facing mental illness and other challenging life situations and will act as a team resource and role model to promote and support positive working relationships and effective team building. They will also provide consultation, clinical assessments, counseling, education, and advocacy in both individual and group settings, with the overarching goal of improving and optimizing mental health and wellness. Required Qualifications Education Master’s Degree in Social Work Current registration and membership in good standing with the College of Social Workers and Social Service Workers Preferred Qualifications The ideal candidate may possess: Minimum 4 years of experience in a health care setting Knowledge, Skills, & Abilities Excellent psychosocial assessment skills Advanced knowledge and application of social work values, ethics, theory, and clinical practices in an outpatient setting Proficiency in evidence-based therapeutic modalities, including CBT and other solution-focused approaches Strong competence in the classification and treatment of psychiatric illnesses such as anxiety, affective disorders, OCD, personality disorders, and depression Experience in planning and delivering social work interventions, both individually and in group settings, with patients, families, caregivers, interdisciplinary teams, and community resources Superior ability to respond to situations using advanced critical thinking skills and a calm, therapeutic approach Thorough knowledge of and ability to access community resources Experience in individual and group education sessions Demonstrated experience in program development and coordination Strong interpersonal and communication skills Ability to work both independently and collaboratively as part of an inter-professional team Demonstrated ability and experience in developing positive working relationships including staff, leadership and other members of the healthcare team Effective prioritization, organization, and conflict resolution skills Effective decision-making and problem-solving abilities Commitment to ongoing professional development Computer literacy, including word processing and database management Experience with Electronic Medical Record systems; EPIC experience considered an asset Dedication to promoting evidence-based, patient-centered care focused on wellness Understanding of consent and capacity legislation Adherence to safety policies and procedures to maintain a safe and inclusive environment for all Empathy in interactions with both patients and staff Commitment to delivering an exceptional experience in a dynamic healthcare setting Proven dedication to demonstrating GHC's values Compensation & Benefits: $67,977.00 - $84,922.50 annually (commensurate with experience) Comprehensive Benefits Package Hospital of Ontario Pension Plan (HOOPP) Paid Vacation Union: CUPE Operations: Monday to Friday – Business Hours Weekends & Holidays Off Applications: Please apply through our ADP Career Centre by May 7, 2025
